Individual and Family Deductible

Dental insurance plans offer individual and family deductibles to accommodate varying needs. Individual deductibles require a specific out-of-pocket payment before insurance coverage applies to dental procedures, while family deductibles consolidate these costs for the entire family's dental expenses.

These plans typically categorize dental work into minor and major procedures. Minor work covers routine treatments like fillings and simple extractions, with insurance sharing the cost. In contrast, major work, which includes procedures such as root canals or crowns, often requires a larger patient contribution. It's important to note that the deductible normally doesn't apply to preventative care, such as check-ups and cleanings, which are often covered at no cost, emphasizing their role in maintaining oral health without additional financial burden.

Your oral health is of utmost importance. With this plan, all preventive services are fully covered from day one, ensuring that you maintain a bright, healthy smile without any out-of-pocket expenses.

Minor Dental Services

We've got your back as your needs change. In the first year, basic dental work is covered at 25%, and in the second year, it increases to 50%. This gradual increase ensures you have the support you need when it matters most.

Major Dental Services

To ensure that our Allstate Dental Vision Hearing PPO Plan remains affordable and accessible to all, major work is not included in the base coverage. However, we understand that some may require additional coverage for major procedures. That's why we offer the flexibility to choose between Level 2 or Level 3 coverage options to meet your specific needs. To tailor your plan and discuss the best option for you, feel free to reach out to a local agent at Utah Avenue. They will provide personalized guidance and help you make an informed decision about your dental, vision, and hearing coverage. At Allstate, we're committed to putting your health and well-being first, offering you the choices you need for peace of mind.

Orthodontia for Children

Please note that orthodontic procedures, such as braces or Invisalign, are not covered under the Allstate Dental Vision Hearing PPO Plan. This decision has been made to maintain lower premium costs and keep the plan accessible and affordable for our members. While orthodontic work is not part of the base coverage, we understand that some individuals may require such services. If orthodontic care is essential for you or your family, we recommend exploring supplementary orthodontic coverage options or discussing potential solutions with an orthodontist to address your specific needs. At Allstate, our primary focus is on providing comprehensive preventive, basic, and essential dental, vision, and hearing coverage to keep you in optimal health.

Implant Coverage

Please be aware that dental implants are not covered under the Allstate Dental Vision Hearing PPO Plan. This exclusion has been put in place to maintain reasonable premium costs and ensure affordability for all our plan members. While dental implants are a valuable and effective dental solution, they are considered a major dental procedure, and our plan primarily focuses on preventive and basic dental services. We encourage individuals in need of dental implants to explore alternative options, such as supplementary dental implant coverage or discuss payment plans with their dental provider to address their specific requirements. Rest assured, the Allstate Dental Vision Hearing PPO Plan remains committed to providing comprehensive coverage for preventive, basic, and essential dental, vision, and hearing care to support your overall well-being.

Maximum Yearly Dental Benefit

Out of Network Limitations and Balance Billing

Some dental plans offer out of network coverage. This is a great benefit because you can visit any dentist in the nation. Which means that if you are traveling, or have a dental emergency, you can visit a dentist for the service you need right away and have a portion of your bill covered by your dental insurance. Please note that if you see an out of network provider, that dentist is allowed to "balance bill" you. This means that your dental insurance company pays your dentist what is reasonable and customary for the services you received. In most situations that dentist accepts that payment and you are only responsible for your portion of the bill. Occasionally, that dentist will decide that the reasonable and customary amount they received from the dental insurance is not enough. That dentist will bill you for the additional charge or 'balance bill' you. You are responsible to pay this amount or contact the dentist to work out a payment plan.
