United HealthCare Dental Vision & Hearing $3000 Benefit Plan

Taking care of your health goes beyond regular medical checkups. Dental, vision and hearing health are just as important to you overall well-being. This DentalWise combo plan with a yearly $3000 benefit provides essential coverage for your dental, vision, and hearing needs. Dental benefits include 100% coverage for preventive services like cleanings and exams from day one. Basic services, such as cavity fillings and simple extractions, are covered at 60% in the first year, increasing to 80% in the second year. Major services, including root canals, crowns, and complex extractions, are covered at 15% the first year, increasing to 50% in the second year. Implant coverage becomes available at 50% after a 12-month waiting period. Implant coverage includes a $1500 lifetime benefit.  Vision benefits cover annual eye exams and offer a $150 hardware allowance for glasses or contact lenses. Hearing benefits include routine hearing exams and up to a $750 reimbursement for prescription or over the counter hearing aids every two years. This combo dental, vision and hearing plan has an annual benefit maximum of $3,000 that resets on the day you enrolled on the plan. While we may not cover implants or orthodontic treatments, we are committed to assisting you in achieving and maintaining a healthy smile through our comprehensive dental coverage.

Orthodontia for Children

It's important to be aware that the United HealthCare Dental/Vision/Hearing 1000 Plan, while designed to offer comprehensive coverage for a range of essential dental, vision, and hearing services, does not include coverage for orthodontic treatments. Orthodontic care, which includes services such as braces and aligners, is a specialized field within dentistry that may require separate coverage or considerations. While orthodontic treatments are not part of this plan, we are here to assist you with your general dental needs and ensure you have access to quality care. Should you require orthodontic treatment, we recommend exploring alternative options or considering supplemental coverage specifically tailored to orthodontics. Implant Coverage

In our commitment to providing comprehensive coverage, we are pleased to introduce implant coverage as part of the United HealthCare Dental/Vision/Hearing 1000 Plan. However, it's important to note that there is a 24-month waiting period for implant coverage from the date of enrollment. After this waiting period, our plan covers 50% of the cost of dental implants, helping you achieve a healthier and more confident smile. We also offer a generous lifetime maximum benefit of $1500 per person for implants, ensuring that you have financial support when you need it most. This coverage reflects our dedication to meeting your dental needs and enhancing your overall well-being.

Maximum Yearly Dental Benefit

Our United HealthCare Dental/Vision/Hearing 1000 Plan offers a maximum yearly benefit of $3000 to help you manage your dental expenses. What's great is that this benefit resets every 12 months from the date you enrolled in the plan. This means you can count on consistent support for your dental needs year after year. Vision Benefit

The vision plan covers your exams 100% and has no waiting period. There is a $10 copay and up to $150 allowance for glasses or contacts when using an in-network provider. Out of network benefits are available as a reimbursement for up to $50 for an eye exam and up to $105 allowance for contacts or $155 for glasses. Included Hearing Benefit

Hearing exams are covered 100% with no waiting period. There is up to a $750 allowance for hearing aids every two years. Out of Network Limitations and Balance Billing

Some dental plans offer out of network coverage. This is a great benefit because you can visit any dentist in the nation. Which means that if you are traveling, or have a dental emergency, you can visit a dentist for the service you need right away and have a portion of your bill covered by your dental insurance. Please note that if you see an out of network provider, that dentist is allowed to "balance bill" you. This means that your dental insurance company pays your dentist what is reasonable and customary for the services you received. In most situations that dentist accepts that payment and you are only responsible for your portion of the bill. Occasionally, that dentist will decide that the reasonable and customary amount they received from the dental insurance is not enough. That dentist will bill you for the additional charge or 'balance bill' you. You are responsible to pay this amount or contact the dentist to work out a payment plan.
