Allstate Select Dental Prime Plan

The Allstate Dental PPO Prime Plan offers a high level of coverage with no waiting periods on covered dental services, so you can start using your benefits right away—often as soon as the next day. This plan includes a $50 deductible per person (up to $150 per family) for in network dentists, and once that’s met, your coverage begins with preventive care covered at 100%. For additional services, the plan uses coinsurance, covering 60% of basic services like fillings in year one and 25% of major services like crowns and root canals after the yearly deductible is met. One of the biggest advantages of the Prime plan is that your benefits increase the longer you keep it. In year two, coverage improves to 80% for basic services and 50% for major services, significantly lowering your out-of-pocket costs over time. This plan also includes orthodontic coverage, paying 15% in year one and 50% in year two, up to a $1,000 lifetime maximum per child 18 years old and younger, making it a great option for families planning for braces or other orthodontic needs. The plan also features a growing annual maximum benefit, starting at $2,000 in year one, increasing to $2,500 in year two, and up to $3,000 in year three and beyond—giving you more coverage the longer you stay enrolled. In the second year and on, basic services are covered at 80% for in network dentists. If you go out of network, then this plan has a $100 per person deductible (up to $300 per family) and covers basic services at 30% the first year and 50% in the second year and beyond. In the unfortunate event that you require extensive procedures like root canals, crowns, or bridges, you can count on us for substantial coverage at 15% in the first year for in network dentists after the deductible is met, providing significant relief for your dental expenses. And here's the exciting part: our support for major work becomes even more substantial in the second year of the plan. in the second year and on, this plan will cover 25% of major services with an in network dentist. If you go out of network, then this plan covers 10% of major services in the first year and 15% the second year and on. Orthodontia for Children

Preventative services include dental cleanings, exams and x-rays. Most plans cover your preventative care every 6 months with no waiting period. Basic dental work includes minor services like filling a cavity and may have a waiting period before these services will be covered on your plan. Major work includes services like crowns and root canals and may have a waiting period before these services are covered. Dentures and implants are normally NOT included in dental insurance. PPO Plans and Out of Network Limitations and Balance Billing

Some dental plans offer out of network coverage. This is a great benefit because you can visit any dentist in the nation. Which means that if you are traveling, or have a dental emergency, you can visit a dentist for the service you need right away and have a portion of your bill covered by your dental insurance. Please note that if you see an out of network provider, that dentist is allowed to "balance bill" you. This means that your dental insurance company pays your dentist what is reasonable and customary for the services you received. In most situations that dentist accepts that payment and you are only responsible for your portion of the bill. Occasionally, that dentist will decide that the reasonable and customary amount they received from the dental insurance is not enough. That dentist will bill you for the additional charge or 'balance bill' you. You are responsible to pay this amount and contact the dentist to work out a payment plan.
