United HealthOne Dental Vision Hearing 1000 Benefit Plan

The $1000 benefit plan is a great all-in-one Dental, Vision, and Hearing option that uses the **large national UnitedHealthcare network**, giving you access to providers across the country. It **covers preventive care like cleanings, exams, and X-rays at 100% right away—before the deductible**, so you can stay on top of your dental health from day one. For basic work like cavities, the plan helps pay **60% in the first year after you meet the yearly $100 deductible, increasing to 80% in the second year**, giving you more coverage over time. It also includes **major services like crowns and root canals**, paying **15% in the first year and 50% after the first year after the yearly deductible is met**, which can really help with bigger, unexpected dental costs. The dental plan includes a $1000 yearly benefit that the plan will pay per person towards your dental services which is enough to cover most regular dental needs. On top of that, the plan includes strong vision benefits with a **$0 eye exam and a $150 allowance each year for glasses or contacts with no waiting period.** It also includes hearing benefits that include a **free hearing exam and a $750 allowance every 2 years toward hearing aids when you use an in-network provider**. It’s a simple, well-rounded plan that helps take care of your dental, vision, and hearing needs all in one place.

Individual and Family Deductible

Dental insurance deductibles work similarly to health insurance deductibles, but they apply specifically to dental care. Individual deductibles are the amount a person must pay out-of-pocket for covered dental services before the insurance starts covering a portion of the costs. In a family plan, each family member has their own individual deductible, but many plans also have a family deductible cap, meaning once the family’s combined spending reaches a certain limit, the insurance will kick in for all members. These deductibles generally apply to services like fillings, crowns, and other restorative treatments, but routine care such as cleanings and exams may be covered without needing to meet the deductible. Understanding how these work ensures you're prepared for any out-of-pocket costs when seeking dental care.

Preventative Care

The preventive benefits on the 1000 plan are designed to help you stay on top of your dental health without worrying about cost. Services like routine exams, cleanings, and X-rays are covered at 100% right away, and the best part is they are covered before you have to meet your deductible . That means you can go in for your regular checkups, catch problems early, and keep your smile healthy without paying out of pocket. It’s an easy, stress-free way to maintain your dental health and avoid bigger, more expensive issues down the road.

Minor Dental Services

For minor or basic dental services, this plan helps cover the most common treatments you may need, like fillings for cavities and simple extractions. After you meet your $100 deductible, the plan pays 60% of these costs in the first year, and that increases to 80% starting in the second year, giving you more support over time . This makes it much more affordable to take care of everyday dental issues, so you’re not stuck paying the full cost out of pocket.

Major Dental Services

For major dental services, this plan helps with the bigger, more expensive procedures like crowns, root canals, dentures, and oral surgery. After you meet your $100 deductible, the plan will pay 15% of these costs in the first year, and that increases to 50% starting in the second year, giving you much more support over time . While you’ll still share in the cost, having this coverage in place can make a big difference when unexpected or more serious dental work is needed, helping reduce those larger out-of-pocket expenses.

Orthodontia for Children

It’s also important to know that this plan does not include coverage for orthodontia, which means services like braces or clear aligners for both children and adults are not covered. If orthodontic treatment is something you or a family member may need, you would want to look at a different plan that specifically includes those benefits. This plan is designed to cover preventive, basic, and major dental care—but not orthodontic treatment.

Implant Coverage

It’s also important to know that this plan does not include coverage for dental implants or any related procedures, such as implant placement or implant-supported crowns . If implants are something you may need now or in the future, you would want to look at a different plan that specifically includes that benefit. This plan is designed to cover preventive, basic, and many major dental services—but not implants.

Maximum Yearly Dental Benefit

This plan has an annual maximum benefit of $1,000, which is the total amount the plan will pay toward your dental care each year . Once the plan has paid out that $1,000, any additional dental costs for the rest of the year would be your responsibility. For many people, this amount is enough to cover preventive care and some basic or major work, especially since cleanings and exams are covered at 100%. It’s designed to help with everyday dental needs while keeping the plan affordable and predictable. If you have a need for multiple major services in the year, you may want to look at increasing the yearly maximum benefit to have that covered.

If implants are something you may need now or in the future, you would want to look at a different plan that specifically includes that benefit. This plan is designed to cover preventive, basic, and many major dental services—but not implants.

Hearing Benefit

This plan has an annual maximum benefit of $1,000, which is the total amount the plan will pay toward your dental care each year . Once the plan has paid out that $1,000, any additional dental costs for the rest of the year would be your responsibility. For many people, this amount is enough to cover preventive care and some basic or major work, especially since cleanings and exams are covered at 100%. It’s designed to help with everyday dental needs while keeping the plan affordable and predictable. If you have a need for multiple major services in the year, you may want to look at increasing the yearly maximum benefit to have that covered.

This is a quick snapshot of the most popular copays, deductibles, and coinsurance that you would be responsible for on this dental plan. Preventative services include dental cleanings, exams and x-rays. Most plans cover your preventative care every 6 months with no waiting period. Basic dental work includes minor services like filling a cavity and may have a waiting period before these services will be covered on your plan. Major work includes services like crowns and root canals and may have a waiting period before these services are covered. Dentures and implants are normally NOT included in dental insurance. If you need coverage for these services, please let your local agent know so that you have a plan that includes those services.

To learn what each of the terms mean, please visit our terms and definitions page. Copays and coinsurance listed are amounts that you would pay after deductible unless specified otherwise. This is not a full list of services included on the plan. This snapshot is not guaranteed to be accurate. To verify plan benefits or for full details please see the full summary of benefits or or get a complete terms of coverage on the health plan carrier's website.

PPO Plans and Out of Network Limitations and Balance Billing

Some dental plans offer out of network coverage. This is a great benefit because you can visit any dentist in the nation. Which means that if you are traveling, or have a dental emergency, you can visit a dentist for the service you need right away and have a portion of your bill covered by your dental insurance. Please note that if you see an out of network provider, that dentist is allowed to "balance bill" you. This means that your dental insurance company pays your dentist what is reasonable and customary for the services you received. In most situations that dentist accepts that payment and you are only responsible for your portion of the bill. Occasionally, that dentist will decide that the reasonable and customary amount they received from the dental insurance is not enough. That dentist will bill you for the additional charge or 'balance bill' you. You are responsible to pay this amount and contact the dentist to work out a payment plan.
