U of U Child Wellness Incentive


University of Utah Health Plans 2022 Child & Teen Well Care Visit Wellness Incentive

The University of Utah Health Plans has expanded their Well Child Visit Incentive in 2022 to include children and teens 3 through 17 years of age.

Yearly checkups are important in keeping kids healthy. It's important for children to visit the doctor, even when not sick, to check things like hearing, vision, growth, and age-related milestones. For your child's first visit to a primary care provider (PCP) for a well care visit in 2022, you will receive a $25 gift card.


You can receive one $25 gift card per child.


A $25 Target gift card is automatically mailed to you once U of U receives the claim for the well care visit. The gift card may be used online or at local Target stores.

The gift card will be sent via USPS in an envelope from the University of Utah Health Plans, so be sure to watch for any mail sent by U of U. University of Utah Health Plans will send the gift card to the address we have on file, and they cannot replace gift cards sent to the wrong address, that are lost in the mail, or that are lost or stolen. Make sure your address is correct! Contact your agent if you need to make a change.


You can receive only one gift card per child, even if the child is seen for more than one well care visit during 2022.


Once your child has been seen for a well care visit with an in-network PCP, his or her office will submit the claim to the University of Utah Health Plans. The claim will be processed and U of U will send you the gift card. You do not need to do anything else for this wellness incentive.


Eligibility for the Gift Cards


To be eligible to receive a gift card, you must meet all of the following:

  • Your child must be enrolled in a University of Utah Health Plan as their primary health insurance coverage at the time of the well care visit.

  • Your child must turn 3 through 17 years of age between January 1st and December 31st 2022.

  • Your child must be seen by an in-network Primary Care Physician (PCP).

  • U of U must receive a claim from your child’s provider for this visit by June 30, 2023. Check out the U of U website for qualifying billing codes.

  • If you or someone else paid for a well child visit out of pocket, this visit does not qualify.

  • Visits to a provider for urgent or emergent care are not eligible for this wellness incentive. Visits to a provider due to an illness do not qualify for this wellness incentive.
