Do you have health insurance through SelectHealth? Then you're in luck!
To help stop the spread of COVID-19 in our communities, SelectHealth is offering a $100 incentive to eligible members on Utah SelectHealth plans who get their full COVID-19 vaccine between August 26, 2021 and December 31, 2021.
Here’s How It Works
Get your shot. Visit any vaccine site and receive your vaccine shot.
Get your second dose, if required. For vaccines that have two doses, you must get both doses to receive the $100 incentive.
Receive your $100 incentive. Once you are fully vaccinated, SelectHealth will send you the $100 incentive.
What Is the Incentive?
The incentive is a $100 gift card. If you are on a Federal Employee Health Benefits (FEHB) plan, your $100 incentive will appear in your Health Incentive Account (HIA) or your Health Savings Account (HSA). If you are on a self-funded plan that is participating in this program, you will be mailed a $100 check.
Who Is Eligible?
To be eligible for the incentive, you must:
Be age 12 or older
Enrolled on a Utah SelectHealth plan.
Complete your full vaccination between August 26, 2021, and December 31, 2021.
